Online Membership Form

Our membershp year extends from July 1st to June 30th

Payments received in June will be applied to the next year's membership.
If you wish to pay by mail, please click the link and print the membership form
and mail it to Lev Eisha of Adat Shalom, 3030 Westwood Blvd., Los Angeles, CA 90034.

For your convenience, you can submit your membership online using PayPal and a credit card. Your payment information will be secure. PayPal is a secure payments organization run by eBay; we will receive the payment but none of your financial information. Please be careful filling out the membership information. It will be e-mailed to the Lev Eisha Membership Chair and Webmistress only. Your personal information will be kept confidential--we do not share our membership list--however we cannot guarantee the security of the internet.

If you would like to contribute an extra double chai ($36) to sponsor the breakfast teachings for the year, please use the custom membership form and add the amount to your total.

You will receive your choice of the following gifts depending on the membership level you choose:
A book chosen by Rabbi August
A custom Lev Eisha heart keychain
The Lev Eisha CD: Shabbat Songs from a Woman's Heart
The Lev Eisha Haggadah, Journey of the Heart
The Lev Eisha Pesach Cookbook, Passover Recipes from the Heart
